目的

分析脑蛋白水解物对痉挛型脑瘫患儿肌肉痉挛评分、脑血流量及血清胰岛素样生长因子-1(IGF-1)、髓鞘碱性蛋白(MBP)、白介素-6(IL-6)的影响。

方法

抽样选取2020年6月至2022年5月湖北省恩施土家族苗族自治州中心医院收治的240例痉挛型脑瘫患儿,依据随机数字表法将其分为对照组、观察组,每组120例。2组均接受相应的功能锻炼康复治疗,对照组在此基础上添加高压氧治疗,观察组在对照组的基础上添加脑蛋白水解物治疗,比较2组临床疗效、肌肉痉挛评分、脑血流量指标[搏动指数(PI)、脑动脉收缩期峰流速度(Vs)、平均血流速度(Vm)]以及血清IGF-1、MBP、IL-6水平。采用二元Logistic回归分析影响疗效的因素。

结果

观察组治疗总有效率高于对照组,差异有统计学意义(97.50% vs 86.67%,χ2=9.659,P=0.002);治疗后,2组患儿肌肉痉挛评分均下降,且观察组患儿肌肉痉挛评分显著低于对照组,差异有统计学意义[(1.16±0.58)分 vs(1.69±0.52)分,t=7.533,P<0.001]。观察组Vs、Vm、IGF-1水平高于对照组,PI、MBP、IL-6水平低于对照组,差异均有统计学意义(P均<0.001)。体质量、粗大运动功能分级(GMFCS)、偏瘫类型、治疗前Vs是疗效的影响因素。

结论

蛋白水解物对痉挛型脑瘫患儿有较好的治疗效果,体质量、GMFCS、偏瘫类型、治疗前Vs是其疗效的影响因素。

Objective

To analyze the effects of cerebroprotein hydrolysate on muscle spasm score, cerebral blood flow and serum insulin-like growth factor-1 (IGF-1), myelin basic protein (MBP), and interleukin-6 (IL-6) in children with spastic cerebral palsy.

Methods

240 children with spastic cerebral palsy admitted in from June 2020 to May 2022 was included. They were randomly divided into the control group and the observation group, with 120 cases in each group. Both groups received corresponding functional exercise rehabilitation treatment. The control group received the hyperbaric oxygen treatment on this basis. The observation group received the cerebroprotein hydrolysate treatment on the basis of the control group. The clinical efficacy, muscle spasm score cerebral blood flow index (PI), cerebral artery systolic peak flow velocity (Vs), mean blood flow velocity (Vm), and serum IGF-1, MBP, IL-6 levels were compared between the two groups. Binary Logistic regression was used to analyze the factors affecting the efficacy.

Results

The total effective efficacy of treatment in the observation group was higher than that in the control group. The difference was statistically significant (97.50% vs 86.67%, χ2=9.659, P=0.002). After treatment, the score of muscle spasm in both groups decreased, and the score of muscle spasm in the observation group was significantly lower than that in the control group, with a statistically significant difference [(1.16±0.58) points vs (1.69±0.52) points, t=7.533, P<0.001]. The levels of Vs, Vm and IGF-1 in the observation group were higher than those in the control group, while the levels of PI, MBP and IL-6 were lower than those in the control group, with statistically significant differences (P<0.001). Body weight, gross motor function classification (GMFCS), type of hemiplegia, and Vs before treatment were the influencing factors of curative effect.

Conclusion

Protein hydrolysate has a good therapeutic effect on children with spastic cerebral palsy. Weight, GMFCS, type of hemiplegia and Vs before treatment are factors influencing its therapeutic effect.
