目的

探讨虚拟现实(VR)技术联合任务导向性训练(TOT)方法对脑卒中多病共存患者功能状态的影响。

方法

选取2019年10月至2021年3月广州医科大学附属第一医院神经内科脑卒中多病共存患者92例,采用随机数字表法将其分为对照组和试验组各46例,分别给予传统康复技术及基于VR的TOT。干预前(病情稳定24 h内)、实施干预1个月及3个月后,于患者门诊复诊时,分别评估2组患者神经功能、Barhtel指数及生存质量改善情况;对共病患者疾病严重程度进行分层,分析不同程度共病对患者神经功能的影响。

结果

试验组干预3个月后mRS评分低于对照组[1(1,2)vs 2(2,3),P<0.001],Barhtel指数评分高于对照组[(72.69±22.58)分 vs(55.97±22.06)分,P<0.001],SS-QOL评分高于对照组[(180.52±33.70)分 vs(171.52±31.72)分,P<0.001],差异均有统计学意义。不同疾病累计评分(CIRS)分层患者的严重指数、共病指数、干预后mRS评分比较,差异有统计学意义(P<0.001)。

结论

基于VR技术的TOT可以促进脑卒中多病共存患者短期内功能状态恢复,改善患者生存质量,值得进一步在临床推广应用。

Objective

To explore the effect of virtual reality (VR) technology combined with task-oriented training on the functional status of stroke patients with comorbidities.

Methods

From October 2019 to March 2021, 92 stroke patients with common diseases in the Department of Neurology of the First Affiliated Hospital of Guangzhou Medical University were included and randomly divided into the control group and the experimental group (46 patients in each group). They were given traditional rehabilitation technology and task-oriented training (TOT)-based on VR technology, respectively. Before the intervention (within 24 hours of stable condition), one month and three months after the intervention, the patients' neurological function, Barthel index, and improvement of quality of life were evaluated at the time of outpatient return visit; the severity of comorbidities was stratified to analyze the impact of comorbidities of different degrees on the prognosis of patients.

Results

After 3 months of intervention, the mRS score of the experimental group was lower than that of the control group [1(1, 2) vs 2(2, 3),P<0.001], the score of Barthel index was higher than that in the control group [(72.69±22.58) scores vs (55.97±22.06) scores,P<0.001], the SS-QOL score was higher than that of the control group [(180.52±33.70) scores vs (171.52±31.72) scores,P<0.001], all with statistically significant differences (P<0.001). There were significant differences in severity index, comorbidity index, and mRS score after intervention among patients with different cumulative disease score (CIRS) levels (P<0.001).

Conclusion

Task-oriented training-based on VR technology can promote the recovery of functional status and improve the quality of life of patients with comorbid ischemic stroke in a short time, which is worthy of further clinical application.
