Research progress on multimorbidity and health management of stroke patients with multiple chronic conditions

Abstract:

Due to the change in people's lifestyle and the aggravation of population aging, the phenomenon of patients with multiple chronic conditions is also increasing gradually. Patients with stroke are often accompanied by disorders of activities of daily life in varying degrees, and the phenomenon of multimorbidity will increase the complexity of health management of stroke patients. As a result, stroke patients have worse health status, higher risk of death, worse prognosis, more hospitalization, higher cost of medical treatment and higher level of use of health management service resources. Therefore, it has increasingly become a major health issue of concern to all sectors of society and research institutes. This article will summarize the prevalence rate, health outcome, and health management status of stroke patients with multiple chronic conditions at home and abroad, to provide a basis for better health management services and practice of stroke patients with various chronic conditions.
