血管内介入治疗是急性前循环大血管闭塞脑梗死救治最有效的方法之一,而介入再通术后即刻头颅CT发现的高密度影有着重要的临床意义,其可能是出血、造影剂增强、造影剂外渗,或者是多种因素并存,尽早识别术后即刻CT高密度影的本质,有助于术后精准用药。另外,有研究提出高密度影是脑出血的危险因素,发现高密度影后及时制定血压调控方案,在一定程度上可以预防脑出血或减轻脑出血的危害,对高密度影进行充分认识有利于指导术后管理。本综述将从研究历程,发生率,出现过程、机制和监测方法,危险因素、分型及其对术后用药的指导、对患者预后的影响,处理措施,预防手段等方面对血管内治疗术后即刻头颅CT高密度影进行阐述。

Intravascular interventional therapy is one of the most effective methods for the treatment of cerebral infarction caused by acute anterior circulation occlusions. And the high-density shadow found by head CT immediately after interventional recanalization has important clinical significance, which may implicate bleeding, contrast agent enhancement or extravasation, the coexistence of multiple factors. Early identification of the essence of immediate CT high-density shadow is helpful for accurate medication after the operation. In addition, studies have proposed that high-density shadow is a risk factor for cerebral hemorrhage. It is found that timely control of blood pressure after high-density shadow can prevent cerebral hemorrhage or reduce harm from cerebral hemorrhage to a certain extent. A full understanding of high-density shadow is conducive to guiding postoperative management. In this review, we will describe the research history, incidence, pathological process, mechanism and monitoring methods, risk factors, classification, guidance of postoperative medication, impact on the prognosis of patients, treatment measures, preventive measures, etc.
